ZIP Coalition Developmental Milestones & Goals

The ZIP-IN Plan envisions a 2030 with highly engaged, effective, inclusive, strategic, collaborative regional coalitions that work in partnership with each other and IDOH to end the HIV epidemic, eliminate HCV, and to ensure that PLHIV and PLHCV have the resources and support they need for an optimal quality of life. In addition to providing guidance and templates, this toolkit includes a basic planning timeline for scaling up support for and capacity of ZIP Coalitions, organized into four areas: Leadership and GovernanceData Collection and ReportingCollaboration and Integration, and Sustainable Funding.

In 2022 ZIP Coalitions participated in the environmental scan process to assess the landscape of community partners and resources and develop regional priorities.

Each ZIP Coalition has a coach who is helping coalitions engage with People With Lived Experience (PWLE), People of Color (POC), and other priority populations. 

ZIP Coalitions are engaged with coaches and receive assistance from the Clinical Quality Management and Community Impact program.

ZIP Coalitions have formed workgroups to address key, regional priorities. For more information see the individual coalition workplans.

IDOH worked with Community Solutions to share data with each of the ZIP Coalitions.

Starting in 2023, IDOH provided ZIP Coalitions with quarterly data reports. It is the goal to be able to share a web-based data dashboard on IDOH’s website so coalitions can filter and review relevant data as indicated by their priorities.

This is in development. IDOH is developing a data dashboard for ZIP Coalitions to utilize.

ZIP Coalitions receive quarterly data from IDOH and review the latest data at each coalition meeting. Data includes: regional demographic data of PLHIV, incidence, prevalence, viral load suppression, retained to care. HCV data reporting is in development.

ZIP Coalitions remain in contact with IDOH staff regarding data needs, concerns, and opportunities.

ZIP Coalitions establish priorities and metrics on an annual basis. These are presented throughout the year at Statewide Advisory Council Meetings.

This is in development. IDOH is developing a data dashboard and quarterly data summaries for ZIP Coalitions to utilize until the dashboard is available.

Ongoing. IDOH Staff, with the Clinical Quality Management and Community Impact program at the forefront, are reorganizing how funding decisions are made using an Impact Focused Funding model. This model leverages ZIP Coalition priorities and needs as well as a variety of data and performance metrics to align funding to need.
